peter e4e334b6e0 simplify FoldingUpdate (IDEA-CR-43672) 
don't cache when applyDefaultState==true (on editor opening)
drop cache when PsiFile changes, which includes the case of its rename, so there's no need to explicitly clear caches then

Aleksey Pivovarov 968432d0a6 diff: set active state before calling ViewerState.init() 
This fixes incorrect presentation of "Switch Diff Tool" action,
as it can use `myState` field when creating toolbar/popup actions.
