7503919a25 fail fast instead of just returning NULL_PSI_ELEMENT to avoid blowing up later and more loudly (e.g. when ActionUpdater starts questioning this null element about containing file like in IDEA-278418 Most actions don't perform after exception in ActionUpdater)

value passed by this parameter represents offset inside line, rather than an index of "column", so it's better to rename it to 'offset in line'

13986b3b7c Fix IDEA-254793: quote the variables in the startup script 
Quotes around SET argument are required for it to properly handle
various special characters in paths: say, ampersand & and braces ().

This will allow out batch script to start IDE from paths containing
these characters.

21bbe22749 remove ApplicationThreadPoolDispatcher in favor of Kotlin's Dispatchers.IO 
- `Dispatchers.IO` is optimized for coroutines;
- it's already used in production, so it doesn't make sense to not use it to avoid another thread pool;
- `IO` is chosen because the application thread pool is unbounded.

3d03f70955 clean up PooledCoroutineContextTest 
- remove `PooledCoroutineContextTest#error must be propagated to parent context if available`, it tests nothing;
- simplify logged error tests;
- fix warnings.
