1. Sync via private obj
2. Primary ctor instead of confusing almost-copy-but-not-ctor
3. getRealIcon() is not synchronized (unnecessary as getRealIcon(ScaleContext ctx) already synchronizes)
4. synchronization includes updateScaleContext now
5. patchResolver -> patch

Tagir Valeev 16f8585588 CachedImageIcon refactoring: 
1. myLocalFilter, myDarkOverridden are final (were practically only changed during initialization, now moved to ctor)
2. trivial getter/setter for myGlobalFilter removed
3. numberOfPatchers removed (unnecessary and even harmful: the same count doesn't mean the same patchers)
4. MyUrlResolver now static class
5. myClassLoader moved to MyUrlResolver and made final
6. unwrapIcon() extracted
7. IconTransform class to encapsulate global icon transformation state (USE_DARK_ICONS+IMAGE_FILTER+ourPatchers)
